Payment Methods:
At 5 Corner Dental accept a variety of payment options such as VISA, MasterCard and Debit card.
At your first appointment, please bring in your dental insurance coverage details as required – insurance carrier, group policy number, certificate or ID number.
Missed/Cancelled Appointments:
If you are unable to attend your scheduled appointment, please provide 48 hours notice to avoid being charged a cancelled appointment fee.
Dental Insurance:
As a courtesy to our patients , our office staff will assist you with your insurance paperwork, however patients are responsible for the balance portion of their treatment costs as well as any treatment procedures that their insurance company does not cover.
